摘 要: 【目的】 建立一种稳定的大鼠宫腔粘连(IUA)动物模型,为深入探讨宫腔粘连的发病机制及治疗提供基础。【方法】 40只健康雌性SD大鼠随机分为对照组(n = 10)及造模组(n = 30);造模组内再随机分为A1、A2 及A3 三个亚组(每组n = 10),造模组刮宫后,行子宫缺血再灌注,宫腔灌注脂多糖溶液损伤子宫内膜。A1、A2、A3 组动物分别于建模后1、2、3个动情周期收集子宫组织,各组动物均于动情期获取子宫标本,行HE和Masson染色,免疫组织化学检测子宫内膜LIF及整合素ανβ3,动态观察子宫内膜病理变化过程,并对损伤后3个动情周期子宫内膜的腺体数目和纤维化面积比率,内膜LIF及整合素ανβ3蛋白的表达水平进行统计学分析。【结果】 子宫内膜再生受损,造模组A1、A2、A3 组大鼠子宫内膜腺体数目分别为3.22 ± 1.21、3.91 ± 1.71、4.12 ± 1.65,均低于对照组8.13 ± 2.31,差异均有统计学意义(P < 0.05),各造模组间差异无统计学意义(P > 0.05)。造模组A1、A2、A3 组大鼠子宫内膜纤维化面积比率分别为0.63 ± 0.05、0.76 ± 0.08、0.79 ± 0.07,均高于对照组0.27 ± 0.04,差异有统计学意义(P < 0.05),各造模组间差异无统计学意义(P > 0.05)。造模组A1、A2、A3 组大鼠子宫内膜LIF蛋白MOD值分别为0.278 ± 0.031、0.293 ± 0.036、0.306 ± 0.036,均低于对照组0.389 ± 0.041,差异有统计学意义(P < 0.05),各造模组间差异无统计学意义(P > 0.05)。造模组A1、A2、A3 组大鼠子宫内膜整合素ανβ3平均光密度(MOD)值分别为0.268 ± 0.035、0.291 ± 0.038、0.301 ± 0.043,均低于对照组0.397 ± 0.036,差异有统计学意义(P < 0.05),各造模组间差异无统计学意义(P > 0.05)。【结论】 采用机械损伤、缺血再灌注及局部感染多重损伤法可以建立稳定的大鼠宫腔粘连动物模型,对子宫内膜容受性造成一定程度的负面影响。
Abstract: 【Objective】 To establish a rat model of intrauterine adhesions (IUA) to benefit the further research on the pathogenesis and pathologic changes and new treatment of IUA. 【Methods】 40 female SD rats were randomly divided into control group (n = 10) and model group (n = 30, further divided into A1, A2, and A3 subgroup, 10/ subgroup). The model group was performed uterine ischemia/reperfusion injury and intrauterine infusion with 0.5 mL lipopolysaccharide solution after uterine curettage. All the animals were sacrificed and obtained uterine specimens in estrus. The A1, A2, and A3 group were obtained specimens at 1, 2, and 3 estrus cycles, respectively after IUA modeling, for observing the endometrial glands, detecting the degree of IUA by Masson staining and evaluating the expression levels of endometrial LIF and integrin ανβ3. 【Results】 Endometrial regeneration was severely impaired by multiple injury, the number of endometrial glands was significantly reduced (3.22 ± 1.21,3.91 ± 1.71,4.12 ± 1.65, to 8.13 ± 2.31) (P < 0.05), and the ratio of the area with endometrial stromal fibrosis to total endometrial area was significantly increased (0.63 ± 0.05, 0.76 ± 0.08, 0.79 ± 0.07, to 0.27 ± 0.04) (P < 0.05). The mean optical density (MOD) value of endometrial LIF for the control group and model group were 0.389 ± 0.041 (control group), 0.278 ± 0.031 (A1 subgroup), 0.293 ± 0.036 (A2 subgroup) and 0.306 ± 0.036 (A3 subgroup), respectively. The statistical analysis indicated MOD values of LIF levels in control group were significantly higher than those in A1, A2, and A3 subgroup, respectively (P < 0.05); and there were no significantly differences between A1, A2 and A3 subgroup (P > 0.05). The MOD value of endometrial integrin ανβ3 for the control group and model group were 0.397 ± 0.036 (control group), 0.268 ± 0.035 (A1 subgroup), 0.291 ± 0.038 (A2 subgroup) and 0.301 ± 0.043 (A3 subgroup), respectively. The statistical analysis indicated MOD values of integrin ανβ3 levels in control group were significantly higher than those in A1, A2, and A3 subgroup, respectively (P < 0.05); and there were no significantly differences between A1, A2, and A3 subgroup (P > 0.05). 【Conclusion】 The multiple injury method of mechanical injury and schemia-reperfusion injury and infection can establish a stable rat IUA model, and it might lead to negative influence on endometrial receptivity in some extent.
