摘 要: 【目的】 研究Roux-en-Y胃旁路术对2型糖尿病大鼠肾脏氧化应激的影响。【方法】 40只SD大鼠,随机分为正常对照组(NC,n = 8),糖尿病模型组(32只),模型组注射链尿菌素(35mg/Kg)成模30只,将其分为糖尿病对照组(DC,n= 8),假手术组(DSO,n = 8),手术组(DO,n = 14)。检测术前及术后8周空腹血糖(FBG)水平,血糖基化终产物(AGE)水平,24 h尿8羟基脱氧鸟苷(8-OHdG)量,尿微量白蛋白(MAU)水平;检测术前、术后1、2、4、8周血清胰高血糖素样肽(GLP-1 )水平;术后8周测定肾质量指数、肾组织中总超氧化物歧化酶(TSOD)、谷胱甘肽过氧化物酶(GSH-PX)、过氧化氢酶(CAT)的活性和丙二醛(MDA)水平,PAS染色观察肾组织病理变化;Westernblot检测各组肾糖基化终产物受体(RAGE)表达水平。【结果】 术后8周DO组FBG由术前的(12.48 ± 1.37)mmol/L降至( 5.45 ± 0.71)mmol/L,术后8周空腹血清GLP-1 由术前(8.12 ± 1.54) pmol/L 升至(22.35 ± 0.79)pmol/L,余各组无明显变化;DC组和DSO组肾组织MDA水平、尿微量白蛋白、尿8-OHdG量、肾组织RAGE表达,血清AGE水平和肾质量指数明显高于DO组和NC组(P< 0.05);抗氧化酶GSH-PX、TSOD 、CAT活性在DC组和DSO组明显低于DO组和NC组(P < 0.05); PAS染色显示DC组和DSO组肾小球细胞外基质积聚,DO组和NC组无明显变化。【结论】Roux-en-Y胃旁路术通过重建肾脏氧化应激和抗氧化应激之间的平衡,从而对肾脏起到保护作用。
Abstract:【Objective】 To investigate the effect of Roux-en-Y gastric bypass on the oxidative stress in the kidney of type2 diabetics rats. 【Methods】 Forty SD rats were randomly divided into normal controlgroup (NC, n = 8) and diabetes model group, n = 32, the diabetes model group wereinjected STZ (35 mg/kg), 30 rates were considered as the successful diabetes model.Then they were randomly divided into a diabetes control group (DC,n = 8),a diabetessham operation group (DSO, n = 8), and a diabetes operation group (DO, n=14). Thelevels of fasting blood glucose ( FBG) before and 8 weeks after operation were tested;serum AGE, 24h urine 8-OHdG were detected after 8 weeksof the operation; glucagon like peptide-1 (GLP-1) was measured before the operation and at 1th, 2th, 4th, 8thweeks after the operation; after 8 weeks of operation the kidney weighty index wasmeasured. The activities of antioxidant enzymes in renal including total superoxidedismutase (TSOD), catalase (CAT), glutathioneperoxidase (GSH-PX)and aleicdialdehyde (MDA) were measured by chromatometry. Western blot analysiswas employed to assess the protein expression of receptor of advance glycation endproducts (RAGE) . 【Results】 The fasting blood glucose of diabetes operation groupdecreased from (12.48 ± 1.37) mmol/L to (5.45 ± 0.67) mmol/L ;the fasting GLP-1 of diabetes operation group increased from (8.12 ± 1.54) pmol/Lto (22.35 ± 0.79) pmol/L 8 weeks after the operation (P < 0.05); the serum AGE,kidney tissue RAGE, 24h urine 8-OHdG, microalbuminuria (MAU), kidney weighty index and MDA increased significantly in the DC and DSO groupscompared with the DO and NC groups, otherwise the activities of TSOD, GSH-Px, CAT decreased significantly (P < 0.05). PAS dyeing indicatesthat glomerulus extracellular matrix accumulating in the DC and DSO groups, theDO and NC groups have no obvious alter.【Conclusion】 The Roux-en-Y gastric bypass surge protects the kidney through re-balancing the oxitive and anti- oxitivesystem.
