Abstract: 【Objective】 To assess the value of ultrasound in evaluating the upper gastrointestine (UGI)of bilious vomiting infantsand comparing it with upper grastrointestinalcontrast study.【Methods】 Thirty-seven infants with bilious vomiting underwentUltrasound examination for excluding upper gastrointestine anomalies were ruled in. Fourteen of them alsoperformedUGIcontrast study. Twenty-three of them also underwent abdominal plain film. Surgery findings or follow up was taken as reference standard. 【Results】 Twenty-three patients(62.2%) were confirmed to have gastrointestinal anomalies

the rest was confirmed normal by follow up. Upper gastrointestinal obstruction were determined in 20 of them by Ultrasound

the sensitivity

specificity and accuracy were 95%(19/20)

76.5%(13/17)

and 86.5%(32/37)

respectively. The sensitivity

specificity and accuracyof UGI in determination of Upper gastrointestinal obstruction were 80%(8/10)

75%(3/4)and 78.6%(11/14)(P = 0.635)

respectively. However

the accuracy in determination of malrotation between US (100%) and UGI (75%) was statistically significant(P = 0.041). 【Conclusions】 The value of US examination in evaluating the upper gastrointestine of bilious vomiting infantsis at least as good as that of UGI

while its value in the diagnosis of malrotation is more valuable than that of UGI.
