【目的】 探讨三维超声造影(3D-CEUS)在原发性肝癌射频治疗中的应用价值。【方法】 128例拟行射频消融的原发性肝癌患者,随机分为两组各64例,观察组为三维超声造影(3D-CEUS)及二维超声造影(2D-CEUS)联合扫描治疗组,扫查方法为术前3D-CEUS扫描,进而采用实时2D-CEUS引导下射频消融;对照组为单纯2D-CEUS引导下射频消融。治疗前观察病灶血管,显示肝癌病灶滋养血管数目及清晰度;治疗2周后复查,比较完全消融率的差异。【结果】观察组动脉期血管的发现率较高,显著高于对照组(206%vs 138%);观察组的完全消融率高于对照组(96.9% vs 84.4%, P<0.05)。【结论】3D-CEUS可以更为清晰的显示原发性肝癌病灶的血管形态及数目,术前联合3D-CEUS有助于提高超声造影引导下射频治疗的完全消融率。
【Objective】 To investigate the usefulness of three-dimensionalultrasound in procedures of ablation for hepatocellular carcinoma (HCC). 【Methods】A total of 128 HCC cases enrolled for ultrasound guided radio frequency ablationwere randomly divided into observation and control groups, each group contain 64cases. Three-dimensional (3D) and Two-dimensional(2D) contrast ultrasound (CEUS) were sequential used in HCC radio frequency ablationin observation group; 2D-CEUS was used in the control group.Before ablation, all the cases were evaluated the number and the clarity of nourishblood vessels inside the lesions; Two weeks after treatment, the difference of completeablation rate between the two group was compared. 【Results】 The combination of 3D-CEUS and 2D-CEUS significantly enhancedthe number of blood vessel detection in artery phase (206% vs 138%, P < 0.001).The combination of 3D-CEUS and 2D-CEUSobtained higher incidence of complete ablation in HCC lesions (96.9% vs 84.4%,P< 0.05).【Conclusion】 3D-CEUS was useful in showing the numberof nourish blood vessel in HCC and combination of 3D-CEUSbefore ablation helps to improve the incidence of complete ablation.
