【目的】 检测URSA患者蜕膜CD4+CD25high Treg中TLR2、TLR4、TLR9的表达,探讨TLRs参与URSA发生的免疫机制。 【方法】选择从2013年1月至2013年11月就诊于广州市妇女儿童医疗中心妇产科的URSA患者,其中B超确认胚胎已停育30例为研究对象,同期选择30例要求人工流产的计划外妊娠的正常早孕妇女为对照组(Control)。收集URSA患者、对照组清宫手术后的新鲜蜕膜组织,流式细胞仪检测两组蜕膜组织中CD4+CD25highTreg 比例以及RT-PCR、Western-blot方法比较蜕膜组织中CD4+CD25high Treg 中TLR2、TLR4、TLR9表达的差异。【结果】URSA患者蜕膜CD4+CD25high T细胞中TLR4、TLR9的表达在mRNA、蛋白水平均下调,差异有统计学意义(P < 0.01),而TLR2表达两组间差异无统计学意义(P> 0.05)。URSA患者蜕膜中CD4+CD25high T细胞低于正常早孕妇女蜕膜(P < 0.01),TLR4、TLR9的表达与CD4+CD25highTreg细胞比例呈显著相关(P < 0.01,P < 0.05)。【结论】 URSA患者蜕膜中的CD4+CD25high Treg减少与TLR4、TLR9低表达相关,TLRs异常表达可能参与URSA的发生。
【Objective】 To detect TLR2, TLR4, and TLR9 expression of decidual CD4+CD25highregulatory T cells in URSA patients and explore the immune mechanism of TLRs involvedin the pathogenesis of URSA. 【Methods】 We selected 30 URSA patients with early embryogrowth arrest by ultrasonography from the Department of Obsterics and Gynecologyat the Women and Children’s Medical Center between January 2013 and November 2013to participate in the study. Meanwhile, 30 healthy pregnant women undergoing electivetermination during the first trimester were conducted as control group. Decidualsamples were obtained from URSA patients with spontaneous abortion and patientswith induced abortion. The proportions of CD4+CD25high Treg in deciduas were calculatedby flow cytometry and RT-PCR and Western blotting were performedto detect TLR2, TLR4 and TLR9 expression of CD4+CD25high Treg in deciduas. 【Results】In deciduas, TLR4, and TLR9 expression of CD4+CD25high Treg in URSA patients weredown-regulated in mRNA and protein levels, and the differenceswere statistically significant (P < 0.01), however, no statistically significancewas found in TLR2 expression of CD4+CD25high Treg between the two groups (P > 0.05).The percentage of CD4+CD25high Treg in deciduas from URSA patients was significantlylower than that from normal pregnant women (P < 0.01), which were significantlypositive correlations with TLR4 and TLR9 expression (P < 0.01, P < 0.05). 【Conclusion】The reduction in decidual CD4+CD25high Treg in URSA patients is correlated withlower TLR4 and TLR9 expressions. Abnormal expression of TLRTLR may involve in thepathogenesis of URSA.
