【目的】 探讨应用多层螺旋CT(MSCT)测量慢性阻塞性肺疾病(COPD)患者气道壁径线与肺功能的关系。【方法】40例受试者分为COPD组(25例)和健康对照组(15例)两组,均进行胸部64层螺旋CT扫描及肺功能检查。CT扫描获取右上叶尖段支气管的横截面图像,测量其管壁厚度(T)与体表面积(BSA)平方根的比值(T/ √BSA)、管腔横截面积(Ai)与BSA的比值(Ai /BSA),并根据上述指标计算支气管横截面总面积(Ao)与BSA的比值(Ao/BSA)、T与支气管横截面总直径的比值(T/D)、管壁横截面积占Ao的百分比(WA%)。分析COPD患者气道壁径线与肺功能的相关性。【结果】COPD组与健康对照组的T/ √BSA[(1.5 ± 0.3) vs (1 ± 0.2) mm/m,P < 0.01]、T/D[(0.3 ± 0.05) vs (0.2 ± 0.02),P < 0.01]、Ai/BSA[(4 ± 1.7) vs (5.6 ± 2) mm2/m2
P = 0.01]、WA%[(81.1 ± 8.1) vs (72.2 ± 4.1)%,P < 0.01]、通气功能(P < 0.05)、小气道功能(P < 0.01)、肺总量(P < 0.05)、残气量(P < 0.01)、气道阻力(P < 0.01)和弥散功能(P < 0.01)差异有统计学意义。T/ √BSA、T/D与通气功能、小气道功能、肺活量(VC)呈负相关(r = -0.724 ~ -0.400,P < 0.05),与残气容积、气道阻力呈正相关(r = -0.378 ~ -0.578,P < 0.05),T/D还与弥散功能呈负相关(r = -485,P < 0.05);Ai/BSA与通气功能、小气道功能、VC呈正相关(r = -0.410 ~ -0.625,P < 0.05);Ao/BSA与通气功能呈负相关(r = -0.425,P < 0.05),与残气量呈正相关(r = -0.497,P < 0.05);WA%与通气功能、小气道功能、VC呈负相关(r = -0.729 ~ -0.445,P < 0.05),与残气容积、气道阻力呈正相关(r = 0.408 ~ 0.488
P < 0.05)。【结论】 MSCT扫描测量气道壁径线,尤其是T/√BSA 、T/D、Ai/ BSA、WA%与肺功能有较好的相关关系。MSCT扫描测量气道壁径线可为评估COPD患者气道病变提供大量的信息。
【Objective】 To investigate the relationship of airway dimensions measured by multi-slice spiral CT (MSCT) with lung function in the patients with chronic obstructive pulmonary disease (COPD). 【Methods】 Forty subjects were divided into two groups: COPD group with 25 patients and control group with 15 healthy subjects. All the subjects underwent 64-slice spiral CT scans at chest and pulmonary function test. The dimensions of apical bronchus of the right upper lobe in the CT images were measured
which included the following parameters: the ratio of airway wall thickness (T) and square root of body surface area (T/√BSA)
the ratio of airway luminal area (Ai) and BSA (Ai/BSA). The following parameters were calculated by the preceding ones: the ratio of the outer area of the bronchus (Ao) and BSA (Ao /BSA)
the ratio of T and the total diameter of the bronchus (T/D)
the percentage of airway wall area to Ao (WA%). The relationship between airway dimensions and lung functions in COPD patients were analyzed.【Results】 There were significant differences between COPD group and control group in T/√BSA[(1.5±0.3) vs (1 ± 0.2) mm/m
