摘要

目的

探讨乳腺癌患者全麻术后恶心呕吐（PONV）的危险因素。

方法

选择乳腺癌全身麻醉患者530例，按照患者术后是否有恶心、呕吐对530例患者的相关病史及麻醉病历分别作单因素和多因素Logistic回归分析，评价乳腺癌患者全麻术后恶心呕吐的危险因素。

结果

530例患者中，发生PONV有101例，发生率为19.06%。多因素分析发现：术前禁饮禁食时间、麻醉维持方式、术中低血压、有无使用止吐药、有无使用曲马多、术后使用阿片类药物镇痛与乳腺癌术后恶心呕吐发生相关（

0.05）。

结论

乳腺癌患者术前长时间禁饮禁食、无使用止吐药、术中低血压、曲马多、吸入麻醉药维持麻醉、术后使用阿片类药物镇痛的术后恶心呕吐中发生率显著增高。

Abstract

Objective

To explore the risk factors for postoperative nausea and vomiting （PONV） in patients undergoing breast cancer surgery under general anesthesia.

Methods

Using univariate and multivariate logistic regression analyses， we reviewed the medical records and anesthesia history of 530 patients who underwent breast cancer surgery under general anesthesia in order to identify the associated risk factors for their PONV.

Results

Among the 530 patients， 101 suffered PONV， with an incidence rate of 19.06%. Multivariate analysis revealed the risk factors associated with PONV occurrence were preoperative fasting， anesthesia maintenance type， intraoperative hypotension， use of antiemetics， tramadol and postoperative opioids （

0.05）.

Conclusion

The PONV incidence is significantly higher in those breast cancer patients with prolonged fasting before surgery， inhaled anesthetics， intraoperative hypotension or no use of antiemetics， tramadol and postoperative opioids.
