摘 要: 【目的】 为了寻找治疗输卵管性不孕的有效方法,探讨人脐带间充质干细胞(WJMSC)治疗输卵管炎的应用前景。【方法】 成年雌性Wistar大鼠50只,随机分为模型组(40只)和对照组(10只)。模型组大鼠双侧输卵管注入混合菌后,再随机分成5组;急性治疗前组(5只)于造模后第20天取材;急性治疗组(10只)于造模第20天,给予腹腔注射WJMSC(1 × 106/mL), 7 d一次,共3次,疗程结束1周后5只取材;慢性治疗前组(5只)于造模后30 d 取材;慢性治疗组(10只)于造模第30日开始上述治疗,1 周后5只取材;慢性模型组(10只): 造模第51天5只取材,余下鼠与雄鼠同笼。比较各组输卵管形态、上皮分泌输卵管特异性糖蛋白(OVGP)、血清INF-α和TNF-?酌水平、受孕率和妊娠数。【结果】 急性期输卵管水肿,上皮坏死,治疗后基本恢复正常。慢性期上皮扁平上皮化生,管壁纤维化,治疗后无明显改善。血清TNF-ɑ、INF-r在急性输卵管炎治疗前〔(1358.75 ± 134.05) pg/mL, (1280.38 ± 240.69 )pg/mL〕、慢性输卵管炎治疗前〔(1335.33 ± 60.35 )pg/mL, (1041.02 ± 146.25) pg/mL〕明显增高,急性治疗后明显下降〔(579.44 ± 47.44 )pg/mL,(843.29 ± 312.42) pg/mL〕治疗前后比较有统计学意义(P< 0.05),慢性组治疗后不能恢复正常。OVGP的分泌在急性输卵管炎治疗前(5.92 ± 3.74)、治疗后(3.15 ± 2.20)与正常组(4 .40 ± 3.84)比较无明显差异(P >0.05);OVGP在慢性输卵管炎治疗前(1.77 ± 1.55)、后(0.41 ± 0.54)和慢性模型组(0.37 ± 0.22)均明显低于正常组(P< 0.05)。受孕率和产仔数在急性治疗组 (100%, (11 ± 3.87)与正常组(100%,10 ± 2.74)比较无统计学差异(P >0.05),在慢性治疗组(20%,2.2 ± 4.92)、慢性模型组(40%,2 ± 2.83)均明显下降,与正常组有统计学差异(P < 0.05)。 【结论】 WJMSC腹腔注射治疗大鼠急性输卵管炎能恢复输卵管的功能,但对慢性输卵管炎疗效差。对输卵管炎的及时治疗是保存生育功能的关键。
Abstract: 【Objective】 This study was to seek effective methods of treating tubal infertility,and discuss Wharton’s jelly - derivedmesenchymal stem cells (WJMSC) application prospect in treating salpingitis. 【Method】50 female Wistar rats were randomly divided into a normal group including 10 ratsand a model group including 40 rats. After intraperitoneal injection of mixed bacteriainto bilateral oviducts, the model group was further divided into 5 groups. 5 modelrats were extracted 20 days after modeled in the ex-treatedacute group; in the treated acute group: from the 20th modeling day on 10 modeledrats received 3 courses (once in a course of 7 days) of intraperitoneal injectionof 1 × 106/ml /WJMSC, 5 of which were extracted one week afterwards and 5 cagedwith male rats; 5 rats were extracted 30 days after modeled in the ex-treated chronic group; for the treated chronic group: 10 ratswere given the same treatment as that of treated acute group from the 30th modelingday on, 5 of which extracted in a week later and 5 caged with male rats; in thechronic model group 10 rats were given no processing, 5 of which extracted on the51th day after modeled and 5 mated with male rats. Then the oviductal morphology,serum TNF-α and IFN-?酌, oviductspecific glycoprotein (OVGP), pregnancy rate and litter size of model groups weredetected and compared.【Result】oviductal damage and edema were observed in acutegroups, but returned to normal after treatment. Epithelial squamous metaplasia andtube wall fibrosis occurred in chronic groups which didn't improve obviously aftertreatment. In acute groups serum TNF-a and INF-r concentration increased significantly before treatment to (1358.75 ± 134.05) pg/mL and (1280.38 ± 240.69) pg/mL, and decreased significantly to (579.44 ± 47.44) pg/mL and (843.29 ± 312.42) pg/mL after treatment, which showed statisticaldifference (P < 0.05); while in chronic groups serum TNF-α and INF-?酌 concentration increased significantly to (1335.33 ± 60.35) pg/mL and (1041.02 ± 146.25) pg/mL before treatment, but didnot restorein chronic groups. There was no statistical difference between OVGP secretion inex-treated (as 5.92 ± 3.74) and treated (3.15 ± 2.20) acutegroup and normal (4.40 ± 3.84) group (P > 0.05). But OVGP secretion in ex-treated (as 1.77 ± 1.55) and treated (as 0.41 ± 0.54) chronicgroup, chronic model group (as 0.37 ± 0.22) were all obviously lower than that ofnormal group (P < 0.05). The pregnancy rate and litter size of treated acute group(as 100%, 11 ± 3.87) showed no statistical difference with that of normal group(as 100%, 10±2.74), but they decreased significantly in both treated chronic group(as 20%, 2.2 ± 4.92) and chronic model group (as 40%, 2±2.83) and were statisticallydifferent from that of normal group (P < 0.05). 【Conclusion】 Intraperitoneal injection of WJMSC treatmenton acute salpingitis rats could recover their oviductal function, but had poor curativeeffects on chronic salpingitis rats. So timely treatment of salpingitis is crucialto preserve the reproductive function.
